Role Overview

The successful candidate will undertake Continuing Healthcare (CHC) and Funded Nursing Care (FNC) assessments for adults with complex health needs. This will include face-to-face assessments across the Birmingham and Solihull areas, with report writing completed either from home or the office.

Key Responsibilities

Complete CHC and FNC assessments for adults with complex health needs
Undertake face-to-face assessments within Birmingham and Solihull
Complete DSTs, Checklists, Fast Track assessments and reviews
Work with patients, families, carers, advocates and multidisciplinary teams
Prepare clear, accurate and comprehensive assessment reports
Support CHC review panels and funding decision-making processes
Manage a complex clinical caseload independently
Liaise with care homes, local authorities, acute trusts and community services
Ensure assessments are completed in line with the National Framework for NHS Continuing Healthcare and Funded Nursing Care

Essential Requirements

Registered Nurse with current NMC registration
Strong experience in CHC / Continuing Healthcare assessments
Experience completing DST, Checklist, Fast Track and FNC assessments
Good understanding of the National Framework for NHS Continuing Healthcare and Funded Nursing Care
Experience working with adults with complex needs, long-term conditions, disabilities, palliative care or end-of-life care
Excellent report writing and communication skills
Ability to work independently and manage a complex caseload
Ability to travel across Birmingham and Solihull for face-to-face assessments

Desirable Requirements

Previous experience working within an ICB, CCG or NHS CHC team
Experience presenting cases to CHC panels
Knowledge of safeguarding, DoLS, Care Act and Mental Capacity Act
Strong stakeholder management and negotiation skills
