Move on and Reconnection Officer – Loughborough
Salary: £ 12.95ph
Full-Time (37 hours)
Job Purpose:
Support individuals identified as rough sleeping or at imminent risk of rough sleeping to move on from temporary to longer term accommodation and reconnect to areas where they are likely to be able to secure longer term accommodation
Offer advice and assistance in relation to housing and finances
Delivery of an effective and appropriate service to all service users, fairly and without discrimination
Responsibilities:
Coordinate the transition of individuals from rough sleeping or temporary accommodation into longer-term accommodation
Identify accommodation and support needs and objectives at the earliest possible opportunity and support individuals to access accommodation and support services
Provide housing advice and signpost and refer individuals to accommodation options and pathways and related support services
Work with Housing Authorities and partners to achieve successful move ons and reconnectinos
Support individuals to access and engage with accommodation and support providers to achieve successful move ons and reconnectinos
Support individuals with their personalised journey of recovery, assisting them to secure and sustain accommodation, maximise income, establish / re-establish and build-up support and recovery networks and improve health and wellbeing
Motivate individuals to identify and achieve accommodation and support objectives and outcomes
Maintain regular contact with individuals and regularly monitor and review progress to ensure objectives are acehived.
Receive and action referrals and co-ordinate the support required to acheive successful move-ons and reconnectons
Effectively manage a caseload and prioritise workload to meet the demands of the service and the needs of customers.
Maintain accurate records of customer needs and support provision ensuring effective and approporate communication across related services and external partners.
Represent Charnwood Borough Council Lettings at case conferences and partnership meetings.
Carry out support and risk assessments on individuals and identify any potential staff or public safeguarding concerns in conjunction with other agencies and organisations.
Assist the CBC Lettings Co-ordinator in the collection and recording of performance management information and the ongoing development of performance measurement techniques for the service.
Assist the CBC Lettings Co-ordinator to monitor and report on interventions and outcomes and ensure that performance targets are met.
Assist the CBC Lettings Co-ordinator to monitor and report on the number and effectiveness of referrals to external accommodation and support providers.
Be familiar with and able to provide advice and guidance with regard to safeguarding adults and children and assist with safeguarding referrals.
Delivery of an effective and appropriate service to all service users, fairly and without discrimination.
To be responsible for protecting and managing information securely, and reporting breaches or suspected information security breaches, in line with Council policies.
