Job Details
Sunderland Care and Support, as part of the Altogether Better Alliance in Sunderland, operates an integrated Health and Social Care Single Point of Access for a range of community based services.
Recovery at Home provides a rapid response and support to people with health and social care needs, enabling people to remain in their own home rather than being admitted to hospital or into long term care.
The service also provides support to people, when they are discharged from hospital, providing a range of solutions to help them to regain their independence and confidence following illness or trauma.
What you can offer
As a Care Coordinator (Level 2) you will help manage and facilitate triaged calls and referrals to support the timely discharge from hospital of customers or the prevention of unnecessary admission into hospital or care.
Suitable applicants must have the ability to deal with emergency situations in accordance with prescribed procedures. You should also be able to provide an excellent professional response to referrals, queries and to respond to high demands at times of surge. Together with the ability to form and promote positive relationships within the team. Whilst also providig support to individuals, their families, colleagues and partner organisations.
Experience is not always necessary; your attitude and values are more important
