Job Details

Please see duties and responsibilities as follows :-

Responsible for placing contract and recording financial cost on Care Track data base contemporaneously for all commissioned care
Administer AQP Service Level Agreements to match the negotiated servicePackages in terms of service provision and Key Performance Indicators

Establish and promote relationships with the full range of stakeholders including, care home owners, domiciliary agencies, social services, and community nursing and acute sector staff.
Requesting copies of care plans 10 working days after commencement of care package
Maintain Care Track (data base system) data recording are robust, ensuring that all data quality is routinely audited to ensure consistent and high quality data.
Ensure that all commissioned providers work within an agreed "Quality Framework and develop where appropriate if not in place.
Investigate provider performance issues, drawing up action plans and monitoring regimes.
To undertake any other duties commensurate with the grade and responsibilities of the post.
Ability to organise and good understanding of communication pathways and the skill to communicate effectively with in a team, key partners i.e. nursing home staff, public, statutory and voluntary organisations
Support the administrator for continuing care providing cover as when necessary
Undertake and comply with all mandatory training.
To undertake any other duties commensurate with the grade and responsibilities of the post.
