Community Care Workers needed for Plymouth City Council, £12.20 - £13.47ph depending on experience

Start Date- ASAP
Contract End date- 5 months initially
Pay rate - £12.20 - £13.47ph depending on experience

Location - Plymouth

Description
To support people in being discharged from hospital who then undertake a period of reablement. During the reablement period people require review to confirm if they are now fully independent and can leave the service, partially independent and would need to leave the service with some support.

What is required?
*Significant experience as a Community Care Worker or similar
*Enhanced DBS
