Adult Social Worker - Continuing Health Care

We are currently looking for a Adult Social Worker - Continuing Health Care to work with one of our clients based in Nottingham. You will be working for the council.

Monday - Friday

Temp contract 

Pay rates - £28ph

Key Responsibilities

To make contact with providers/care homes/agencies to gain information and evidence to support the LA review process and ensure they are keeping significant records to support the CHC application as robustly as possible
To work alongside NHS Berks CHC team and associated outsourced CHS agency, and be confident to challenge as necessary to ensure the best possible outcomes for the Council are achieved
To complete Local Authority review documents assessments and reviews to support the CHC process
To participate in Health led CHC MDT – Multi Disciplinary Team meetings
Knowledge of Care Director would be preferable
To liaise with families and carers to explain the process
To maintain case records and to fulfil Service procedures and statutory duties
To maintain an up to date spreadsheet of all referrals, actions and progress on each individual
