Hospital Discharge Social Worker

We have an opportunity for Adult Social Workers to work with the Hospital Discharge Team at Worcester Royal Hospital. This is a temporary role for 3 months with potential to lead to a more long-term position. You will be required to work Monday to Friday, 8:30 am to 4:30pm. The role is to ensure that the outcome of all work with adults in need of care and support, and with their carers, is to promote their wellbeing.

Responsibilities:

* Liaising with a team of social workers and case coordinators

* Undertaking assessments

* Participating in team meetings

* Arranging and implementing care plans

* Facilitate discharged from A&E and CDU

* Arranging urgent support and intervention to client in community to avoid hospital admissions

Hourly Rate: £20.81 - £23.81
