Job Details

Job Purpose

To give one-to-one support to mental health service users to work towards their recovery and help them to live independent and fulfilling lives. The role is based in the community and largely involves working with people in their own homes. The majority of people using the service do so through purchasing it via their social care personal budget. The role also includes supporting the individual to use their budget by writing and implementing their support plan. The role may also include providing support in other areas such as group facilitation. The service is based around a person’s needs and the role therefore requires evening and weekend work.

Main Duties and Responsibilities

Casework

  1. To assist with the provision of a wide-range of practical support services for clients. This may include helping them to get out into their local community such as going to courses or voluntary positions, pursuing interests, using local shops and attending appointments. The role also includes supporting individuals with correspondence, helping them maintain or regain skills, offering companionship or enabling their usual carer to take a short break from their caring responsibilities.
  2. Identify concerns or risk and follow agreed procedures.
  3. Motivate individuals to help them manage their lives to achieve their maximum potential.
  4. Take a team approach, working collaboratively with other people involved in the individual’s care and support to plan and deliver person centred support for service users.
  5. To undertake regular risk assessments and create a risk management plan for each client on their caseload.
  6. To monitor a client’s recovery by using the Outcomes Recovery Star tool and maintain a record of it online.
  7. To write and review the service user’s social care support plans and put the agreed plan into place.
  8. Recognise any changes in the service user’s health and care needs and communicate these to Sheffield Mind and the relevant Community Mental Health Team.
  9. To implement the tasks detailed on the support agreement.
  10. To attend client reviews and CPA meetings and other appointments as necessary.
  11. To liaise where necessary with other relevant professionals, agencies, informal carers, families and volunteers.
  12. Provide cover for other Support Workers as required.
  13. To participate in and assist the organisers of activities, courses and group work.

Administration

  1. To keep appropriate records.
  2. To maintain necessary casework records.
  3. To write reports as required.

General

  1. To act at all times in accordance with Sheffield Mind's aims, philosophy and policies.
  2. To work in accordance with Sheffield Mind's equal opportunities policy.
  3. To treat all information in accordance with Sheffield Mind's confidentiality policy.
  4. To undertake training as required.
  5. To undertake any other reasonable duties which are consistent with the objectives and nature of the post as agreed by the post holder and the Board of Trustees.
  6. To participate in regular supervision with line-manager.
  7. Signpost to other services where necessary including other Sheffield Mind services.
  8. Maintain appropriate professional boundaries.
  9. Represent Sheffield Mind as necessary, promoting its values and reputation.
